Food Health and Fitness

Dr. John Poothullil, MD, FRCP, is a board-certified physician, award-winning author, and nationally syndicated health expert focused on the root causes of chronic disease. He practiced for more than 30 years as a pediatrician and allergist, including 27 years in Texas, before retiring from clinical medicine in 2008.

Over the course of his career, Dr. Poothullil developed a deep interest in the biology of hunger, metabolism, and weight gain—launching a decades-long independent research journey across biochemistry, physiology, and endocrinology. His work led him to challenge conventional thinking around insulin resistance and propose an alternative model for Type 2 diabetes, recently published in the peer-reviewed journal Medical Hypotheses.

He is the author of multiple books, including Diabetes: The Real Cause and the Right Cure, Eat Chew Live, and the award-winning Your Health Is At Risk, which received Gold medals from both the IPPY and Nautilus Book Awards.

Dr. Poothullil has published research and articles on hunger, satiation, weight loss, diabetes, and the senses of taste and smell in leading medical and scientific journals, including Physiology & Behavior, Neuroscience & Biobehavioral Reviews, Journal of Women’s Health, Journal of Applied Research, Nutrition, and Nutritional Neuroscience. His work has also been featured in major consumer publications such as Woman’s Day, Fitness, Redbook, and Woman’s World.

He is a nationally syndicated contributor to The Price of Business and a frequent podcast and media guest, known for challenging conventional health narratives while offering practical, science-based strategies to prevent and address lifestyle-related diseases.

He is based in Portland, Oregon.

Medical Misinformation vs. Science (how to evaluate conflicting health advice)

Lifestyle Diseases (prevention and reversal through practical interventions)

1. You talk about an “authentic weight.” What does that mean—and why do most weight-loss approaches fail long term?

2. The idea that insulin resistance causes Type 2 diabetes is widely accepted. What led you to challenge it—and what did you find instead?

3. Your recent work builds on the Randle Cycle and the idea of fuel competition in the body. Can you explain how glucose and fat compete—and why that matters for diabetes?

4. You’ve described what you call the “fatty acid burn switch.” How does this concept fit with metabolic science, and how does it change the way we understand high blood sugar?

5. If Type 2 diabetes is driven by nutrient overload and fuel competition, what is the most important first step someone should take to reverse it?

6. With the rise of GLP-1 drugs like Ozempic, many people are turning to medication for weight loss and blood sugar control. How do these treatments fit—or conflict—with your metabolic model?

7. You’ve been critical of mainstream dietary guidelines. Where do you think agencies like the U.S. Department of Health and Human Services may be overlooking key metabolic principles?

8. Your cookbook translates your science into daily life. How should people actually eat if they want to avoid “fuel competition” and keep blood sugar stable?

9. You’ve said much of what we’re told about nutrition and chronic disease is incomplete or misleading. What are the biggest myths people still believe?

10. Many patients feel dismissed when they question standard medical advice. How can people advocate for themselves while still working constructively with their doctors?
